CHILDREN'S NASAL FLU VACCINES
We now have nasal vaccines available for two and three year olds (children aged 2&3 years of age on 31st August 2017)
Two children's vaccination clinics are currently available and you will need to book your child/children into these clinics via our Reception.
ADULT FLU VACC APPOINTMENTS available on:

As part of the initiative to provide better support to patients the practice has provided a named GP for all of its patients. The named GP is, where necessary and appropriate, responsible for coordinating a patient's care with other health professionals.
You are able to see any GP you would like at the practice and are not restricted to just this one GP. If you would like to know who your allocated GP is please ask at reception.
If you have a strong preference as to who your named GP is, please let the practice know and we shall try to accommodate your request.

WORKING TO AVOID UNPLANNED ADMISSION TO HOSPITAL
NHS England and Welton Family Health Centre will be reviewing patient information in order to identify those patients most likely to be admitted to hospital. This is a national scheme called "Avoiding Unplanned Admissions" where the intention is to pro-actively manage the care of t hose patients identified as being most at risk and pre-empt an unplanned admission to hospital.
The information gathered is for the Practice use only and only the GPs and Practice Staff will be involved however, should you wish to opt out of this risk profiling process then please let a member of the Practice Staff know.

Online Access Info
Once you have completed and signed the necessary forms please bring them into surgery together with photographic proof of your identification we will be able to provide you with a personalised username and password. You can then access SystmOnline by clicking on the following link or from the link on the home page.
Repeat Prescriptions - order your repeat medication online
As part of the SystmOnline service you can order your repeat prescriptions using this service. You will need a username and password to access as outlined above.
Accident and Emergency
Please could patients NOT use A&E as an alternative to seeing a GP – there are enormous pressures on all A&E services and we are working with our Clinical Commissioning Group to try and reduce any unnecessary attendances. If patients are unsure about attending A&E they should speak to the Duty Doctor for advice.
Patients are encouraged to use NHS resources wisely. The A&E Department at Lincoln County Hospital is available for Accident and Emergencies ONLY.
There are several other avenues of expert help and advice available, both in and out of hours:
NHS Walk In Centre - to treat minor illnesses and injuries, no appointment needed. Vist the Walk In Centre, 63 Monks Road, Lincoln, LN2 5HP.
Call 111 - if it's not an emergency but you need medical help fast.
A&E or 999 - for emergency care only.
Before you decide to go to A&E please think if one of the other options would be more appropriate. Thank you.
